Social Security Number

The Taxpayer Relief Act of 1997 requires educational institutions to obtain a valid Social Security Number (SSN) or Individual Taxpayer ID Number (ITIN) from all US Citizens and Permanent Residents who choose to or are required to file a US tax return.

Students who provide a SSN/ITIN may be eligible to receive an educational tax credit for qualified expenses for courses taken at UC San Diego Extended Studies. Students who do not provide a SSN/ITIN may be subject to a fine by the IRS. If you do not have income that is subject to US tax, you are not required to supply this information to us. However, please be advised that UC San Diego Extended Studies will report all student records to the IRS whether or not you have provided this information. Legal Name

The principal purpose for requesting personal information, such as legal name, is to enroll you as a student with UC San Diego Extended Studies. Your legal name will be displayed on official university documents, such as your academic transcript and 1098T, and in systems that must reflect legal name due to legal or regulatory requirements.

If you wish to be called by a name other than your legal name, please provide that information in the lived name field below. If provided, your lived name will appear in UC San Diego Extended Studies systems such as class rosters, Canvas, and My Extension.

Lived Name

You may provide your lived names to be used on certain UC San Diego Extended Studies records and systems. This includes first name, middle name, and last name.

Your lived names will appear to staff and instructors in UC San Diego Extended Studies documents and systems such as class rosters, Canvas, and MyExtension (Please note that lived name is in the process of being implemented and some systems are still being modified. We thank you for your patience as we make these upgrades).

UC San Diego is required to list your legal name for all licensure agreements, state/federal requirements, or any legitimate business needs (i.e. OSHA courses). If the receiving institution and/or third party requires documents (e.g. transcripts, financial aid/records, 1098T-tuition-tax information) with your legal name, please leave the “lived name” (formerly referred to as preferred name) field in your MyExtension student portal blank and your legal name will be used on all University documents. Your legal name will continue to be used on official university documents and in systems that must use legal name due to legal or regulatory requirements.

Extended Studies reserves the right to remove or deny a lived name submission if used inappropriately. This includes, but is not limited to, names using foul or inappropriate language or names submitted to avoid legal obligation or to create misrepresentation.

Personal Pronoun

You may indicate a set of gender pronouns by which you would like to be identified. You may choose from a list of commonly used pronouns or specify a set of custom pronouns. Personal pronouns will be displayed on the class roster and the student information systems used by Extended Studies staff to provide you with assistance. We reserve the right to remove or deny a pronoun if used inappropriately.

Military Affiliation

1098-T Electronic Delivery

Federal regulation allows those eligible to receive IRS 1098-T form, the option to receive the form in an electronic format instead of through mail.

By selecting electronic delivery, you are consenting to receive all 1098-T tax forms electronically. Your consent will remain valid for all subsequent years, unless withdrawn. You may withdraw consent to receive a 1098-T electronically at any time by changing your election via the MyExtension portal. If you have any questions regarding this election, please contact Student Services at unex-1098t@ucsd.edu.

1098-T for International Students

By selecting electronic delivery, I hereby acknowledge that I wish to receive my 1098-T tax form electronically. I understand I will no longer receive a paper version of my 1098-T and my consent will remain valid for all subsequent years, unless I change my election via the MyExtension portal.
